In-person Candidate Information
Candidate Fee $70.00 Includes (1) candidate dinner ticket, a social baldric, 4th Degree Lapel Pin, and an Exemplification Certificate. Form 4 with fee should be sent to the Oklahoma District Executive Assistant SK J. J. McCarthy. Dinner ticket for guest $25.00.

Dinner Reservations
Everyone (except candidates) needs to make a dinner reservation. Contact SK Steven Ackerman (Faithful Navigator), for the dinner tickets for all guests. Seating may be limited and not available after the June 15, 2022, deadline.
Other than candidates, all Dinner Reservations are $25.00 payable to: Archbishop Salatka Assembly 2044

Official Dress for Candidates
All CANDIDATES, (except Clergy and Military) must wear a dark suit, black tie, black socks, black shoes, with white shirt with a flat laydown collar (no button-down collars). Vests and wing-tip shirts are NOT permitted.
All MEMBERS should wear official dress with social baldric or regalia. Military, Police & Firemen may wear Class “A” or Dinner Dress Uniforms.
All Candidates
Form Four (4) and Candidate Fee of should be sent to the Executive Assistant for the Oklahoma District Master, SK J. J. McCarthy no later than June 17, 2022 after approval by the Council and Assembly. All applications must be properly completed including the membership number and email address.
