SERVICE AWARD FORMS

Print out one form for each Service Program.

Fill out all sections, use additional paper if needed.

Please include as much detail as possible, explain why you feel that your activities best exemplify each service program. If possible add pictures, showing brothers, families and other volunteers in action. You can also include newspaper clippings, newsletters, pamphlets, etc.

Winners of each service program will be forwarded to the Supreme Council for competition in the International Level so presentation is key!

Describe in detail, your candidates accomplishments in their community, family and parish life. Letters of recommendation, Acceptance Letters, pictures, newspaper clippings, etc. are highly encouraged.

KNIGHT OF THE YEAR

Describe in detail, your candidates’ accomplishments in their community, family, council and parish life. Letters of recommendation, pictures, newspaper clippings, etc. are highly encouraged.
