Memorial Request Form
Every member should have a Memorial Form on file with the Assembly. To submit your Memorial request, click on Memorial Form, print and complete the form. Please note that your request can be either 'Yes' or 'No'. Also note that your spouse should sign the form. Return your completed form to Faithful Captain Robert Fragnito either in person at a meeting, or scan and send it electronically to Robert Fragnito. Thank you.
