The Form 4 application and banquet reservation forms will be available on the 4th Degree Illinois District 1 website. You can fill out the application for membership using the online fillable form then print sign and get it hand delivered to Tim Cosgrove Financial Secretary for the Joliet Assembly.
Complete BOTH the application for 4th Degree membership and the reservation form with check and get to Tim.

The 4th Degree Exemplification the Joliet Assembly participates in will be next February, You can fill out a Form 4 Application by the end of the year. You can attend any Exemplification of Patriatism though without waiting for the one next Feb.
