FOURTH DEGREE MEMBERSHIP APPLICATIONS:
Membership Applications (Form 4) may be obtained from any Assembly, Council, District Master or Secretary to the Master. Applications must be COMPLETED IN FULL AND SIGNED BY the following: Candidate, Proposer, Faithful Navigator, Faithful Comptroller and the Council Financial Secretary. Failure to obtain all necessary signatures will delay the processing of the application.
IMPORTANT: The application Form 4 (all copies) should be attached to the completed reservation form provided, along with a check payable to Bill Schmidt, Kentucky District Master to:

There will be NO refunds made after this date

EXEMPLIFICATION FEE:
The fee, set by the Supreme Council, for Fourth Degree Candidates (including Deacons) is $70.00. The fee includes a lapel pin, social baldric and one banquet ticket (whether you attend the banquet or not). It does not include hotel accommodations. There is no exemplification fee for Priests but it is requested that their Council or Assembly pay for his banquet ticket. This fee does not include the yearly dues payable to your assembly.
LUNCH:
This is an optional box lunch before the Exemplification available for the Candidates and Sir Knights. It will be served at 12:00 p.m. There is a $5 fee, payable with registration. There will be very few tickets available on the day of the Exemplification. Please reserve your ticket with your registration.
BANQUET TICKETS:
Tickets for the Banquet are $30 if purchased by October 1. Tickets purchased after that day will be $45 until October 12. Any tickets purchased after that or at the door will be $60.
ATTIRE: **PLEASE READ - NEW UNIFORM**
Candidates and Sir Knights must wear a Dark Suit (Black or Navy Blue), Plain White Dress Shirt – Button cuff (no French cuffs – no button down collar shirts), Conservative Necktie, Black Socks and plain Black Shoes. The new uniform is to be worn as you would a business suit.
Priests and Deacons may wear their clerical attire. Military Candidates may wear their full dress uniform. Police, Firefighters, and Emergency Medical Technician Candidates may wear their dress uniform.
Color Corps and File Leaders must wear the new uniform or black tuxedo, black bow tie and cummerbund, plain white pleated front shirt with a lay-down collar (no wing collars, frills or color), black socks and black shoes with the respective regalia. While the preference is for the new uniform wherever possible, the old regalia can still be worn until June 30, 2019. After that date, only the new uniform should be worn.
Faithful Navigators, Past Faithful Navigators, Former Masters, Former Vice-Supreme Masters and Color Corps Commanders may wear the regalia of their current, past or former office, including the jewel of their current Council or Assembly office, or Miniature Jewel(s) should be worn.
HONOR GUARD/COLOR CORPS:
Honor Guard and Color Corps will be under the direction of District Marshal Gary Dykhuis. Sir Knights possessing Color Corps regalia may dress for the Mass procession. Formation for Mass will be at 4:45pm.
HOTEL ACCOMMODATIONS: A block of rooms has been set aside for this exemplification at the Clarion Hotel, 1950 Newtown Pike, Lexington, Kentucky. Each individual is responsible for making his own room reservation directly with the hotel. Reservations must be made by September 21, 2018 by calling 859-233-0512 (please refer to group KofC 4th Degree). Our group rate is $119 + tax per night for a Standard Room (non-smoking hotel with two queens or king bed). The rate is based upon availability and includes a breakfast buffet and Wi-Fi. Check-in time is 4:00 pm ET.
LADIES SOCIAL & INFORMATION:
The Ladies will meet at the Cathedral of Christ the King by 10:00 a.m. The social will include a tour of a local Lexington historical landmark and lunch. The Ladies Social should conclude by 2:00 p.m. More details will be sent once the Ladies Social is finalized.
Ladies attire for the Mass and Banquet is semi-formal. Remember, your Sir Knight will be in a suit or tuxedo. Business Casual clothing is appropriate for the luncheon.
